Service Description: A comprehensive program designed to help individuals manage and prevent Type 2 diabetes which provides one-on-one and group consultations with certified diabetes educators, including registered nurses and dietitians. Topics covered include blood sugar monitoring, medication management, nutrition, exercise, and lifestyle choices. Operates across multiple locations in Lambton County, including Forest, Watford, Kettle and Stony Point First Nation, and Sarnia.
