Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurses, dietitian, and social worker- mental health and anxiety programs * one-on-one counselling and therapy
- arthritis rehabilitation and education program * occupational therapist coaching and therapy * in-home or in-office * Arthritis Society of Canada sponsored program (open to everyone)
- asthma and chronic obstructive pulmonary disease (COPD) programs * education and self-management
- nutrition and lifestyle programs
