Service Description:

Family and Transition Support Services is designed so families can
choose the type of support they need. Navigating government funding processes,
connecting with community resources and transition planning through key stages
in life are a few of the services we can provide.- adults with a disability

Services provided:
- assist with the
management of government funding, facilitate purchases and complete
financial documents for clients who have passport funding
- making connections and
navigating resources in the community
