Service Description: Offers a variety of community based exercise classes and education with the goal of promoting self-management of chronic conditions, maintenance of physical fitness and improvement in overall quality of life for the community. Programs include:
  • Therapeutic Exercise
  • Upper Body Exercise
  • Respiratory Wellness
Placement in any of the programs, including virtual or in-person and start times is dependent upon an assessment completed prior to the start of the program

 Therapeutic Exercise Programs: In-person therapeutic circuit or virtual therapeutic group classes led by the Physiotherapist, Registered Nurse, Registered Kinesiologists and Physiotherapy Assistants. This program is intended for adults with functional or mobility limitations requiring modified and supervised exercise due to a chronic health condition. Clients work on strengthening, cardiovascular endurance and balance exercises.

Upper Body Exercise Program: In-person and/or virtual group exercise program led by Occupational Therapists. This program is intended for adults with mild-severe upper body limitations due to chronic health conditions. Exercises are mostly done from a seated position and are designed to help clients improve the use of their upper body through weight-bearing, stretching, isometric exercises, strengthening, coordination and fine motor activities.

Respiratory Wellness Program: Virtual group exercise and education program led by the Registered Respiratory Therapist, Registered Kinesiologist and Physiotherapy Assistant. This program is intended for adults managing a chronic pulmonary disease. Clients work on strengthening, cardiovascular endurance and balance.

Meetings: Classes run for 1 hour twice per week either in-person or online pending assessment.

Eligibility / Target Population: Adults aged 25 and older who: • Require exercises to be modified and supervised • Are able to work in a group exercise setting • Are able to bring and independently administer emergency medications (i.e. rescue inhalers, Nitro) • Can stand from sitting with minimal assistance and be able to stand with support if needed for a minimum of 15 seconds (for in-person programs) • Can follow instructions and communicate any adverse effects of exercise • Have functional limitations or chronic pain that severely limits their ability to perform ADLs (for the Therapeutic Exercise classes) • Have decreased functioning of the arm or hand (for the Upper Body Exercise classes) • Have a respiratory condition that limits their ability to perform ADLs (for the Respiratory Wellness class) • Clients should not be participating in Outpatient Rehab Services through HDGH or in other community exercise programs while participating in CDMP exercise programs
