Service Description: 125-bed long-term care home * residential care for 63 private rooms and 62 semi-private rooms * 24-hour nursing and personal care * access to a doctor and other health professionals

Adult Day Program

LEVEL OF CARE:
Light Care - Can manage most activities of daily living, may require cueing or supervision; requires assistance with provision of basic needs, i.e. medications, nutrition, hygiene, housekeeping, etc.
Medium Care - May be able to manage some activities, but requires assistance and supervision with activities such as transfer, mobility, personal care, medications, nutrition, etc; mobility and cognitive status variable
Heavy Care - Requires total care with most or all activities of daily living with complex care nursing needs and/or behavioral management difficulties.

SPECIAL UNITS: 1 secure unit for dementia care/those at risk of wandering

BATHROOM TYPES: Ensuite (private in private room, shared in semi-private)

TOUR FREQUENCY: By appointment, Mon-Fri preferred

STRUCTURE TYPE: 3 stories of resident living areas

FACILITY SECURITY TYPE: 1 secure unit, with access to secured therapeutic gardens

RESIDENT CARE SERVICES: Nursing: catheterization (intermittent, indwelling, suprapubic), condom drainage, inhalation therapy, oxygen, suctioning (deep, oral), recreation therapy, restorative care, wound care

FACILITY LIFESTYLE - PERMISSIONS: smoking area (outside of building), medication is administered by registered staff

FURNITURE POLICY: Personal furnishings are encouraged, provided they fit safely into the room. Facility bed must be used for resident and staff safety

MEAL PLAN / SERVICE TYPES: Dining room in each resident area, special diets including diet type and texture, beverages served between each meal, snacks in the afternoon and at bedtime

PHYSICIAN VISIT FREQUENCY: Residents have their choice of attending physician. Medical director on site 2 days per week

RESTRAINT METHOD: Least restraint policy, lap belt

Fees: Rates set by the Ontario Ministry of Health and Long-Term Care * subsidies for basic rates may be available for eligible applicants
